SmartThing Phone APP created a new Home location?

I recently purchased and replaced my old phone with a new one and installed the SmartThings app on the new phone.

When I go into the app under the menu Home, I see 2 Homes. The one I always used and I am guessing a 2nd one from the new phone.

How do I get rid of the Home I don’t want.

Also, my SmartThings does NOT recognize that my phone is away from home now. I am suspecting it is because the new phone is somehow tied to the 2nd Home I can delete.

Your help is greatly appreciated.
