(That thread says “orphan” because that’s what SmartThings support calls it, but you’re right, it’s not an orphan (A child device that cannot find a listed parent), it’s a ghost (the parent, in this case the zwave controller, cannot find a listed child).
Ghosts typically occur when a device is removed but the address table does not get updated correctly. It can happen. It’s more likely to occur in a cloud architecture because then you have multiple copies of the device table.)
