The serial number on my hub that is having the same zwave/zigbee problem looks just like yours in the web app - its a 32 character UUID instead of the 10 digit number shown on the bottom of the hub. I never looked at the serial number in the advanced web app before to know if the actual number was there before.
I see you have multiple hubs on 000.058.00010 firmware - are the others v2 hardware and working correctly?
