Using the new ST app, open the SmartApps tab, click the … on the top right, Tap delete, select the app you want to uninstall and tap Delete.

I don’t have a delete option on my IOS version. Just edit and then it allows me to hide and rearrange apps on my home screen.

Click on the menu (3 bars in the upper left of the screen) select Smartapps, then click on the three dots in the upper right of the screen and choose delete and select the smartapp you wish to delete.
