This is just z-wave devices, I have not surveyed sensors. I have a version 2 hub.
What is going on with the z-wave stack that is causing this? I’ve tried removing and repairing devices and this hasn’t helped.

Have you run a Z-Wave network repair lately? If you look at devices in the IDE do you see “Unknown Device” when looking at an offline device’s route?
yes, i don’t have any ghost devices.
lost a thermostat this morning…
the routing done by z-wave repair is dump. hub is in middle of house, z-wave repair will route from one side of house to the other and then to the hub.
