Pairing a Monoprice Z-Wave Plus Smart Plug and Repeater with 2 USB Ports

New to SmartThings and Monoprice Z-Wave Plus devices (though had a previous controller from another company and a number of Z-Wave devices). I seem to be having problems pairing the Monoprice Z-Wave Plus Smart Plug and Repeater with 2 USB Ports device with my SmartThings Hub and have tried following the instructions both here via the manufacturer and the included instructions that came with the device.

I am hoping that someone on the forum has had luck with this device and can recommend some things to try to pair it correctly via the app (iOS). Thanks in advance for any suggestions as to what I might try to get this to successfully pair!
