It’s a GE Zwave Dimmer Model 12724
It’s relatively new, but I’ve been using it for a couple weeks. I didn’t notice it doing this until today. I have an app that re-purposes the “off” button as a toggle for other lights and it worked fine until today. Now when I turn the main lights off it triggers the app because of this duplicate event being sent.
This is happening with the stock “Dimmer Switch” device type.
It’s weird, it seems to happen pretty much at exactly 7 seconds after the first “off” event. Here’s another log:
