I created custom icons, but you can use the built-in SmartThings icons by their full url.

I’ve received a report that my SmartApp does’t work on Windows Phone, but I’m still trying to figure out if it is just the one user or all Windows Phone users. I don’t have a Windows Phone so it’s been difficult to troubleshoot.
